Singer/guitarist Kurt Cobain wrote "Drain You" about his bittersweet relationship with Bikini Kill member Toni Vail. During this time, Kurt was beginning to find out that although the punk community is toted as being a place where individuality is celebrated and there were no rules, in reality people were very discriminate against each other and there were many "rules" to follow. This song is about both those issues in his life.

"Gallons of Rubbing Alcohol Flow Through the Strip" is a bonus track, labeled a "devalued American dollar purchase incentive track", available on European, Mexican and Australian copies of In Utero, as well as various other non-U.S. pressings. It is a jam recorded in Rio de Janeiro, Brazil in January 1993, and does not get a separate track position on the disc, starting at about 23 minutes after the end of "All Apologies".

"Lake of Fire" is actually out of Kurt Cobain's vocal range. He said he wanted to struggle with the vocals and he was said to be very proud of his vocals on the song after all was said and done.

In 1992, MTV had requested Nirvana perform their smash hit "Smells Like Teen Spirit," while the band themselves had indicated they preferred to play their new songs, "Rape Me" and "Tourette's". This did not go over well with network executives, who continued to push for "Teen Spirit." Finally, offering a compromise, MTV executives offered the band a choice to play either "Teen Spirit" or "Lithium," which the band appeared to accept. However, at the actual performance, Nirvana began to play, and Kurt Cobain played the first few chords of the song, "Rape me" much to the horror of MTV execs. The band did finally return to their regular performance of "Lithium." Near the end of the song, frustrated that his amp had stopped functioning, bassist Krist Novoselic decided to toss his bass into the air for dramatic effect. Unfortunately, he misjudged the landing, and the bass ended up bouncing off of his forehead, forcing him to stumble off the stage in a daze.

Guns N' Roses vocalist Axl Rose challenged Nirvana vocalist Kurt Cobain to a fight during the 1992 Video Music Awards, after Cobain and his wife, Courtney Love, egged him on. Additionally, in the end of Nirvana's performance on the VMAs of the same year, while Cobain was trashing the band's equipment, drummer Dave Grohl ran to the mic and shouted "Hi, Axl! Where's Axl?" repeatedly.

The band was going to name themselves "Pen Cap Chew" until they decided to call themselves "Nirvana" before debuting on March 1988.

"Polly" is actually based on a real-life event Kurt Cobain read about in the newspaper. A girl in his homestate of Washington was kidnapped and raped, but by seeing her captor as a human, she managed to befriend him and escape. The story struck Kurt hard, and he felt compelled to write about it.

"You Know You're Right" was one of the last songs Nirvana wrote and recorded before Kurt Cobain's death in 1994. It wouldn't make it onto an album for another eight years.
